The Medical Form - take 2

So... our medical form was completed at our Dr. visit two weeks ago BUT we didn't get the patient ID number in the correct places so it has to be redone, then renotarized, then re-apostilled. PLEEEEEASE pray that everything will be filled out correctly (again) and that it will all be acceptable for the Ukrainian side of things. We are holding on to a few things that need to be apostilled until we get the medical forms (and are given the ok on those) before we send our remaining documents to be apostilled. Its cheaper to do them all at one time instead of one at a time. We are only a little more than 2 months from our submission date and I'm getting antsy to have everything DONE and ready to FedEx to our translator.

I am very thankful that we decided to start gathering documents way ahead of time because we have the time to fix things like this that we didn't forsee being an issue.

We had to re-do our medical forms too, ugh what a pain. I pray you will have it all in order in no time :)

It is a big pain isn't it? I hated having to call our Dr. office and telling them, "Hey guess what?"
We've done the employment letter THREE times as well...
